Residential Habilitation
A New Leaf is happy to announce that we are now accepting applications for individuals wishing to receive Residential Habilitation Services.
A New Leaf has direct care staff that provides services hourly and 24 hours a day/7 days a week in individual’s homes. We provide individualized services and supports encouraging participant choice, providing the greatest degree of independence possible, enhancing the quality of life, and maintaining community integration and participation. Services provided are intended to be person-centered and participant-driven, and based on a person-centered plan to meet each participant’s needs for self-sufficiency, medical care and personal development with goals that safely encourage each participant to become a productive member of the community in which they live. We provide oversight, training, and quality assurance for every direct care staff member employed.
A New Leaf has an initial review process where we will determine if we can meet the individual needs of the participant and to determine if the participant qualifies for Residential Habilitation, under Medicaid provided standards and guidelines.
